THIS IS A BACK-UP DOCUMENT in case the student is unable to get both the student and partent/guardian signatures on the NOMINATION AGREEMENT in online application. This document should be SUBMITTED BY THE NOMINEE when filling out the online application.

BOTH student AND parent/guardian signatures must sign the agreement.
